Required Student Documentation to include with the Application- Birth certificate
- Current physical exam and immunization record (must be fully immunized as required by the State of Delaware. Please note that we do not accept religious exemptions)
- Baptismal certificate (if applicable)
- Most recent report card and standardized test scores (last two years of report cards and tests for grades 2-8)
- Psycho-educational evaluation (if applicable)
- Teacher recommendation form
